Requires an AA or Bachelor’s degree in human services field.  Experience working with families and community agencies.  Forty (40) hours of agency training relevant to: Healthy Families project goals, services, policies and operating procedures; relationships with other community resources; successful completion of State sponsored Healthy Families core training (can be completed post-hire). Use of personal vehicle is required.

Energetic individual needed to provide assessment, planning, linking, and monitoring of services to children with serious behavioral health problems in Bay County.  Obtain collateral information from service providers, complete assessments. Review service/treatment plans. Assist eligible recipients with the implementation of the service plan to meet identified needs. Link the recipient with specified services and resources identified in the service plan. Coordinate service delivery for recipients. Monitor service delivery to recipients. Facilitate recipient's access to needed services. Provide continued assessment of recipient's needs and resources to update service assessment and service plan as needed. Maintain client related documentation in accordance with Center policies and procedures, component specific guidelines, Medicaid and contract provisions as applicable. Initiate and participate in activities for staff, program and Center development. Requires bachelor’s degree in human services field and one year’s experience working with children with serious emotional disturbances.
